For Patients

To prioritize the well-being of our immunocompromised patients, OAOC has implemented a mandatory mask policy for all individuals visiting our facility. This measure is in place to provide a safe and protective environment for everyone under our care. Your health and safety are our top priorities, and we appreciate your understanding and cooperation in adhering to this policy.

If you happen to have any symptoms like a fever, cough, flu-like symptoms, or any respiratory discomfort, we would like to kindly request that you consider not visiting our office. Instead, we warmly encourage you to get in touch with us to reschedule your appointment for a telemedicine visit. Your health and safety, as well as the well-being of our staff and fellow patients, are of great concern to us, and we genuinely appreciate your understanding and cooperation in this regard.

New Patient

We appreciate you scheduling an appointment with our office. To ensure your visit is as productive as possible, we suggest jotting down any concerns or important matters beforehand. This will help us prioritize and address them with the utmost care and thoroughness. Please understand that certain autoimmune conditions may require multiple office visits for proper diagnosis and treatment, but rest assured, we are dedicated to providing you with comprehensive care and support throughout your healthcare journey. Thank you for entrusting us with your health.

First Visit

  • Please complete the Patient Registration forms and bring them with you to your first visit in order to reduce your wait time.

  • Contact any doctor offices you have been to in the last year and have them fax us any medical records. Alternatively, you may want to pick these records up and bring them yourself.

  • Bring a copy of your insurance card with you, as well as any co-pay that may be due. We will check at the time of your appointment to see if you have any out-of-pocket cost, if you do, we will require payment at that time.  We accept cash, check, money order, and VISA/MC only.

Important Information

  • We accept most insurances. The exception to this is that we are not a participating provider with DSHS (medical coupons) or with any form of basic or Apple Health. We ask that you contact your insurance to make sure that our Physicians (Arinola Dada or Kent Ta) are participating with your plan.

  • Our office requires that you have a primary care physician (PCP). It is your responsibility to check with your insurance prior to your visit in regards to any specific requirements they may have for you to see a specialist. If a referral is required, and not here, the visit may be your full financial responsibility.

  • We can only reschedule your New Patient appointment once. Cancellations require 24 hours’ notice or you will be charged a no-show fee of $100.00 for all appointment no-show/no-call including any follow-up visits.
